There is a small right pleural effusion. Which other structures are most strikingly abnormal?

A pacemaker is in place but the heart is not enlarged. There is minor shadowing of the right lung base adjacent to the pleural effusion, but elsewhere detail of the lungs is reduced by the abnormal bones. These are very dense and have a mottled texture.

This patient had prostate cancer with sclerotic bone metastases.
