Rotation - spinous processes (orange line) not central between clavicles (green lines)

Inclusion/Inspiration/Penetration - adequate

Massive right pleural effusion with - 'white out' - 'meniscus sign' (arrow)

Mediastinal shift? - assessment difficult due to rotation - but the right main bronchus is displaced (asterisk)

Cannot comment on heart size - as right heart border not visible

Chest drain in situ (red tube)

Large pneumothorax (*) - no evidence of tension

Residual effusion with 'meniscus sign' - one edge appears horizontal (orange line) due to direct interface of pleural fluid with air

Correct term therefore 'hydropneumothorax'

Cannot comment on heart size - as right heart border not visible
